I believe you're referring to this part?

Thread a yarn needle with white yarn and insert it through round 23 at the bottom of the head, bringing it out at the inside of the right eye, between rounds 9 and 10. Wrap the yarn around the back of the safety eye and insert your needle back into the same stitch it came out of. Bring your needle back out in round 23, one stitch away from where your yarn entered into the head. Pull on both strands and tie them together. Repeat these steps for the left eye and weave in all yarn ends.

Basically, what you do here, is attaching a thread to the safety eye and pulling it inside the head. You do so for both eyes and then make a knot, so it cannot loosen. This will give the face a little bit of shape, but it is not necessary as the unicorn is equally cute without doing so.

So you enter through the bottom of the head, go to the safety eye, attach the thread around the eye, then go back to the bottom of the head and pull it.
